ABSTRACT

Rosai-Dorfman disease or sinus histiocytosis with massive lymphadenopathy is an idiopathic, benign, histiocytic proliferative disease involving lymph nodes and extranodal sites. A 52-year-old woman presented with a 2-year history of multiple non-tender, erythematous nodules on her face. The initial punch biopsy showed lymphoid follicle-like aggregation of CD20 positive lymphocytes in the papillary dermis consisting of primary cutaneous marginal zone B-cell lymphoma. She underwent a 3-month course of radiotherapy, but the lesions progressed. The re-biopsy specimen revealed marked histiocytes with positive CD68 and S-100 proteins. The patient was treated with prednisolone and isotretinoin to which she responded slightly. Repeated histopathologic examinations enable us to make a definite diagnosis. Differentiating these two disorders as early as possible is crucial for appropriate treatment.

ABSTRACT

Mucoepidermoid carcinoma is the most common primary salivary gland malignancy. About 60% of mucoepidermoid carcinomas occur in the major salivary glands and 35% in the minor glands, in which the palate and buccal mucosa are the most frequent sites. Histologically, tumor cells, composed of intermediate, mucous, epidermoid, and clear cells in widely varying proportions, form numerous sheets, islands, duct-like structures and cysts. Many foci of mucin production are prominent. Herein, we report a case of a 37-year-old man who presented a nodule on left buccal mucosa. After the diagnosis, he was referred to a dental surgeon, by whom the tumor was radically excised. But, tumor cells were observed at one of the resection margins. As a result, he received radiation therapies (6 weeks, 60 Gy). He is now undergoing regular follow ups without any signs of recurrence.

ABSTRACT

Doxifluridine is a pyrimidine derivative and is activated to 5-fluorouracil by pyrimidine phosphorylase. Multiple acral hyperpigmented macules have been reported in patients treated with systemic 5-fluorouracil or some of its prodrugs. However, there have been no reports of this adverse event being induced by doxifluridine. Herein we present a 42-year-old woman with multiple pigmented lesions on the palm and sole after chemotherapy with oral doxifluridine.

ABSTRACT

BACKGROUND: Lichen sclerosus et atrophicus (LS et A) is an uncommon chronic lymphocyte-mediated dermatosis that causes substantial discomfort and morbidity, most commonly in adult women. It occurs predominantly in the anogenital area, and extragenital lesions also occur. OBJECTIVE: The aim of this study is to compare clinical and histopathologic features of genital and extragenital LS et A. METHODS: We retrospectively reviewed the medical records and 31 biopsy specimens of 29 patients who had been diagnosed with LS et A between January 2001 to June 2007 in Asan Medical Center. RESULTS AND CONCLUSION: The male to female ratio of genital LS et A and extragenital LS et A was 1:20, 1:3 respectively. Mean age at onset of genital LS et A and extragenital LS et A was 47.8 and 36.9 respectively. The color of the lesions was mostly white, irrespective of the involved area. Genital LS et A lesions were mostly located on labium majora or minora. Extragenital LS et A lesions were mostly located on the trunk, head and neck. The predominant symptom for genital LS et A was pruritus, whereas extragenital LS et A was mostly asymptomatic. The main treatment regime of LS et A was topical corticosteroid. The combination therapy with topical corticosteroid and pimecrolimus/tacrolimus resulted in symptomatic relief as well as improvement of skin lesions, without significant side effect. Histopathologically all LS et A cases showed vacuolar alteration, papillary dermal hyalinization, and dermal lymphohistiocytic infiltration. Hypergranulosis, exocytosis, red blood cell extravasation, lichenoid inflammatory cell infiltration were more frequently observed in genital LS et A (p<0.05). Follicular plugging, epidermal atrophy and loss of rete ridges were more frequently observed in extragenital LS et A (p<0.05).

ABSTRACT

Xanthogranuloma is a benign, fibrohistiocytic lesion that presents as a yellowish, cutaneous papule or nodule. Although the head, neck, and trunk are the most common sites for xanthogranuloma, it can appear at any site on the body. We report a rare solitary xanthogranuloma on the palm of 24-year-old man.

ABSTRACT

Cutaneous metastasis from bladder cancer is very rare. A 48-year-old man presented with a 5 month history of a dome-shaped nodule on his scalp. Histopathologic examination of the skin lesion revealed the proliferation of atypical cells, which were similar to those of urothelial carcinoma of the urinary bladder. We herein report on a case of cutaneous metastasis from urothelial carcinoma of the urinary bladder with a review of the Korean literature.

ABSTRACT

Lichen sclerosus et atrophicus is a benign, chronic lymphocyte-mediated dermatosis that causes substantial discomfort and morbidity, most commonly in adult women. It has predilection for the anogenital region, but extragenital lesions also occur, mainly on the upper trunk, axillae and buttocks, as slightly sclerotic white scar-like lesions with a wrinkled surface. Extragenital lesions are clinically similar to genital lesions, but due to their rarity and lack of symptoms, clinicians often make a misdiagnosis. We herein present a case of extragenital lichen sclerosus et atrophicus, which began on the upper back and spread to other areas displaying typical wrinkled, white scar-like lesions.

ABSTRACT

Verrucous carcinoma is a slowly-growing, well-differentiated, low grade squamous cell carcinoma with little tendency to metastasize. There have been several case reports of this tumor but its occurrence at a young age is extremely rare. Herein we report a case of verrucous carcinoma arising on the anogenitalia of a young woman with pancytopenia.

ABSTRACT

Doxepin hydrochloride cream with potent H1 and H2 blocker activity is a tricyclic antidepressant, which is structurally similar to phenothiazines, and also known to be a contact sensitizer and photosensitizer. We report a case of allergic contact dermatitis due to doxepin hydrochloride cream in a 75-year-old-man, who developed facial edema and eczema at the application sites (face, neck and upper trunk) within several hours of application of doxepin hydrochloride cream. Clinicians should be aware of the possibility of allergic contact dermatitis to doxepin cream, if the condition worsens with use of this medication.

ABSTRACT

Mucormycosis is an opportunistic fungal infection frequently associated with diabetes mellitus or an immunocompromised status. Rhinocerebral mucormycosis, the most frequent form of mucormycosis, is characterized by facial pain, swelling, fever, blood-tinged rhinorrhea and shows a rapidly progressive course. Herein, we describe an immunocompromised and diabetic woman with rhinocerebral mucormycosis initially misdiagnosed as herpes zoster.
